Health Screening Questionnaires
All MPharm students are required by law to undertake Health Screening
- Please read the information leaflet explaining how you should complete the forms.
- Please ensure that your name and other unique identifiers (e.g. date of birth, etc.) are properly filled in on ALLsections of the form. Take your form to your General Practitioner (GP) and have them complete the questionnaire - please note that some medical practices can take up to eight weeks to complete this process, and our Occupational Health Unit require the forms at the start of the term.
- Please contact your GP's practice today and make the necessary arrangements. Students will not be able to complete compulsory parts of the course without completing this process. International students should see their own medical practitioner and have these forms completed prior to leaving for the United Kingdom.
- Once your GP has completed their section then please check again that your name and date of birth is stated on all pages of the questionnaire (in the appropriate sections) and then hand your completed form into our Occupational Health Unit at: The Occupational Health Unit, Liverpool John Moores University, 68 Mount Pleasant, Liverpool L3 5UX during your first week at University.
