Support and care home services for people aged 75 plus
Research in this area focuses on evaluating services and care schemes to prevent unscheduled admissions to hospital in people aged over 75 years, either as the result of falls or through the effective management of long term conditions. The research programme has been developed in Cheshire as part of an Innovations Forum project.
